I know this thread is pretty dead, but...

I just did a version using a covered battery holder from Radio Shack and a host cable bought from iriver. The battery holder is a AAA one with a cover and an on/off switch. I tested it out using a Kingston Technology card reader. Works fine with 128MB and 1GB SanDisk cards. Doesn't work with the 16MB canon card that came with my camera.

Instead of cutting into a separate extension cable I cut into the host cable and attached the negative side of the batteries to the black wire and put a single 1N4001 diode on the positive side, between the batteries and the red wire in the cable. I didn't cut through the wires, just scraped away a spot on the insulation and soldered to it. You have to be careful not to short anything to the braided shield wires.

I was able to fit it all in the extra space in the battery holder. After getting everything soldered I cut the side of the battery holder and used epoxy to hold everything in place.

I used NiMH batteries, which are a little lower voltage than alkaline, so I'm not sure if the voltage would be a little high with alkaline. With freshly charged batteries the voltage was 5.02 volts at the start. By the time I'd transfered 325MBs, the voltage was down to about 4.8V.
